Anamnesis sheet for clinical environmental medicine

First things first. Anamnesis takes time. Your time and help. The success of our diagnosis largely depends on your willingness to cooperate and honesty when filling out the anamnesis form and the timeline. Please take the time (approx. 1 hour) to fill out these forms and send them to us in advance.

Please note:

The completed anamnesis form and the timeline must be submitted to us electronically at least one week before the first appointment. It forms the basis of our work and is the prerequisite for effective cooperation.

If questions arise or points remain unanswered, these will be discussed with you in the first appointment.
